Postherpetic Neuralgia PHN


– Hi everyone. This is Ryan Church from the
Comprehensive Spine Center. A patient wrote in today and asked about postherpetic neuralgia, or PHN. This is a condition that is very painful and can be actually devastating
to people who have it. It’s the chronic pain that
happens after the shingles virus or herpes zoster virus has run its course and it truly is a
condition where a patient has permanent nerve damage
and can feel a wide range of severely painful sensations in the area where they had shingles, and one patient asked, “Why does it never go away?” And one of those, the reason is really
because it’s nerve damage, and the nerves in the area are sending aberrant signals, pain
signals, to the brain. It’s really quite distressing. But there are some treatments
for postherpetic neuralgia. You can try Lidoderm patches, which are by prescription only. There are some medications such as Gabapentin, Lyrica, other
neuraleptic-type drugs that can help reduce painful symptoms in postherpetic neuralgia. Also, spinal cord
stimulation is also used, which is an implantable device in and around the spinal
column to help scramble or interrupt painful
signals going to the brain. And I’m also aware of other therapies that are kind of emergent technology, such as Calmare therapy, can also be somewhat helpful. I’ve seen it be helpful in patients with postherpetic neuralgia. So there are some things that can be done. A good pain management expert, someone who is board
certified in pain management, can be consulted for the
best treatment options for postherpetic neuralgia. I really wish you good luck with that. That can be very debilitating. Hope that helps.

  1. I've had PHN for over 6 months and tried Lyrica, Nucynta, topical pain cream, lydocaine patches, and am currently on Gabapentin and having acupuncture treatments. Nothing works.

  2. I've had PHN for almost 6 YEARS (!) and absolutely cannot bear it without gabapentin. I started out on 100 mg every 8 hours and now take 600 mg every 8 hours which is not working the entire 8 hours now. The gabapentin keeps the constant pain at a level I can bear. I have tried other pain relievers in conjunction with gabapentin for the frequent, excruciating stabbing pain but nothing works for that pain. My shingles outbreak involved the upper left side of my face and scalp and this area is supersensitive now to touch – even a tiny breeze on my hair can stop me in my tracks. The area is also bumpy, hard to heal and itches like nothing I've ever known. In the last 5 plus years since I had shingles, I have lost everything I ever owned and have all but lost my mind. I was I was 53 yrs old when I had shingles. Before the shingles, people often guessed my age as early 40s. I will soon be 59 and look more like 69. My life isn't and will never be what it could have been.

  4. Very educational video, well done. And yeas, anyone can get rid of zoster, or shingles within as little as 3-5 days. By using a remedy based on a few key ingredients, it’s very simple to do it at home. Maybe just try googling Kaena Ramingler, and see if her suggestions agree with you.

  5. I had an attach if shingles on 20% of my body ( half of front area down my stomach to my groin area, my back from my bra strap area to top of my butt). Still have swelling around my waist area. Anyone else suffering from swelling?

    I'm taking 2400 mg of gabapentin, ladocaine ointment but I have to melt a half tsp to put on the painful area because I can't rub it in as it's semi hard state.

    I got shingles on Sept. 21, 2018. I spent Thanksgiving and Christmas ? covered under a blanket because I couldn't deal with the pain.

    My doctor started me on gabapentin & it was 2400 mg that allowed me to cope with the pain.

  6. Thanks for the video, I have had PHN for a year and half. I think it might have left me (one week without feeling like a cripple).I don't want to jinks myself. It is horrible and yes I have done the Lycodine patches but I never could put them on properly as I was treating my psoriasis and the soft creams would make the patches fall away. I am not sure I am clear of the PHN but I hope I am I hope you and your followers feel better. It's horrible and not many people talk about it so good for you.
